The hole comes from air bubbles in ice cubes which may gather together and form a bigger hole.

You can separate the 

ice cubes connected 

to each other using a 

shovel or something.

These are the chips from the protrusion or the corner of ice cubes.

You can remove them with a shovel if that bothers you.

For hygienic reasons, replace the water 

in the water tank once a week.
